Two of our homes are located in Fremont, and three in Dublin. Each home offers the support and care your loved one needs. Our diverse care team is qualified to provide exceptional care to adults with intellectual disabilities of all levels. They will receive compassionate care, mutual respect, and will be provided with positive life learning opportunities.
Each of our houses is located in an established neighborhood and provides a home environment with individualized care.
We have an professional staff that includes Registered Nurses, Occupational Therapists, Physical Therapists, Recreational Therapists, a Dietician and Pharmacist. The services for a Psychologist and Psychiatrist are included when needed.
Whether your loved one needs ambulatory care or is wheelchair bound, our team is here to help. Each person residing in our homes receives individualized, person centered services developed by our team using input provided by our residents and their families.
Staff are onsite 24 hours, 7 days a week. All consumers will receive a minimum of one hour of nursing services per week.
Community Day Program is the first of our programs to open in Fremont over 30 years ago.
Sierra Day Program was opened in Dublin in 2006.
These programs offer person-centered plans that match each individual's interests and goals. Experiences are designed to build on essential community skills and increase self-sufficiency through engaging training sessions. Participants will develop abilities like following instructions and completing multi-step tasks. They will also regularly engage in community activities to help boost their confidence and independence, empowering them to thrive in various settings.
We provide door to door transportation for the following areas: San Lorenzo, Dublin, Livermore, Pleasanton and San Ramon, Union City, Hayward, and Castro Valley.
